Dog Training Participation Acknowledgment & Liability Waiver

By purchasing or registering for training services provided by Primacy K9, I acknowledge and agree to the following:

Health & Vaccination

I certify that my dog is in good health, is free of contagious illness, and is physically able to participate in training activities. I confirm that my dog is current on vaccinations recommended by my veterinarian, including rabies as required by law.

I agree not to bring my dog to class if my dog is showing signs of illness, including vomiting, diarrhea, coughing, or other symptoms of contagious disease.

Behavior Disclosure

I agree to disclose any known behavioral concerns, including any history of biting, injuring, attempting to bite, or displaying aggressive behavior toward people or other animals.

I understand that failure to disclose behavioral issues may result in removal from class without refund if the trainer determines the dog presents a safety risk.

Assumption of Risk

I understand that dog training activities involve interaction with other dogs and people and carry inherent risks. These risks include, but are not limited to:

• dog bites or scratches
• dog fights or altercations
• unpredictable animal behavior
• injury to people or dogs
• property damage

By participating in training services, I voluntarily assume all risks associated with participation.

Responsibility for My Dog

I understand that I am solely responsible for my dog's behavior at all times during training classes, private lessons, or related activities. I agree to maintain control of my dog and to follow all instructions provided by the trainer.

Dog-to-Dog Injury Responsibility

I understand that while dogs typically run one at a time in these classes, dogs participating in training may be present in proximity to other dogs.

If my dog injures another dog or causes damage to property, I agree that I am financially responsible for any resulting veterinary or repair costs.

I further agree to indemnify and hold harmless Primacy K9, its owners, employees, assistants, contractors, volunteers, and facility hosts from any claims arising from my dog’s actions.

Release of Liability

To the fullest extent permitted under California law, I release and hold harmless Primacy K9, its owners, employees, assistants, contractors, volunteers, and facility hosts from any and all claims, liabilities, damages, injuries, losses, or expenses arising from participation in training services.

Removal from Class

The trainer reserves the right to remove any dog from training if the dog presents a safety risk to people or other dogs or if the handler is unable to maintain control of the dog.

Removal due to undisclosed behavioral issues may occur without refund.

Photo & Media Release

Unless I notify the trainer otherwise in writing, I grant permission for photographs or video taken during training sessions to be used for educational or promotional purposes.

Electronic Agreement

If any provision of this agreement is held to be invalid or unenforceable, such provision shall be modified or severed to the minimum extent necessary so that the remaining provisions remain valid and enforceable.

By completing my purchase and registering for training services, I acknowledge that I have read, understand, and agree to this Training Participation Acknowledgment & Liability Waiver, and that this electronic agreement is legally binding.
